This form is required to be completed by the team manager, of the team whom the spectator is affiliated with and whom was ejected. You are required to gather information from spectator or from the rink management.

Home Team reminder - Any spectator ejection during a game, needs to be notated on the the official scoresheet, with the period/time, and name if known.
